360 bed secondary care hospital serving 150,000 population in Hawke’s Bay. Teaching Hospital for medical students of University of Otago (Wellington Clinical School).  Hawke's Bay is a major wine growing region in NZ with over 2,200 sunshine hours per year.  Registrars can enjoy a range of leisure and outdoor activities.

Description of Service

Organisation
Staff: 15 General Physicians with subspecialty interests (Cardiology 3, Geriatric Medicine 4, Respiratory 2, Gastry 2, Diabetes 2, Acute Medicine 2) share acute on call plus 1 Renal Physician, Neurologist, Rheumatologist, Dermatologist and Palliative Care Physician
Units: Single General Medical Service including Acute Admitting Unit, CCU and Acute Stroke/Neurology Unit. All under department well supported by Emergency Physicians, ICU and a comprehensive radiology service.

IM Specialty Interests / Special Units: Cardiology 3 specialists, CCU, Echo & Catheter Lab, Geriatric Med 4 specialists and ATR unit Respiratory 2 specialists and full Resp Lab, Diabetes/Endo 2 specialists, Infectious Disease, Neurology, Gastroenterology 2 specialists and full endoscopy service Renal Medicine, Neurolog and Rheumatology 1 specialist each.

Formal Training

Lectures: Weekly department educational session - registrar run for physicians, registrars and HO. Weekly registrar tutorial - specialist run
Journal Club: Weekly – specialist run
Grand Rounds: 1 weekly, cross specialty
Consult Rounds: Minimum 3 x weekly and evening acute round
Specialty Rotations: 3-6 month rotations negotiable in general medicine runs with subspecialty experience in Cardiology, Respiratory Medicine (with bronchoscopy and respiratory lab experience), Gastroenterology (with endoscopy experience), Diabetes and Endocrinology, Renal Medicine, Acute Medicine Unit and Infectious Diseases.
Trainees in General Medical units also participate in OPD clinics in Neurology and on trainee request in Rheumatology and Dermatology.
Research: Active research department participating predominantly in cardiology and stroke trials
Quality Improvement: Monthly department audit session

Trainee Responsibilities in IM

Coverage Roster: 13 medical registrars share the 24 hour on call rota including nights (split into 4 [M-Th] and 3 [F-Su] 1 in 13 each. 10 registrars are attached to GM on call teams, each with subspecialty interest (2 Cardiology, 2 Acute Medicine, 3 Geriatric/Stroke, 1 each Gastro, Respiratory and Diabetes/Endo), 1 Renal, 1 holiday relief and 1 night relief.
Workload in Medicine Units: Average 24 admissions per day, shared amongst all teams (min 4) rounding.  3 x ward rounds per week plus 11-2 OP clinics per week.
Teaching Responsibilities Yes, to junior medical staff/nursing/students
Research/presentations: Average of one presentation every 3 months to weekly dept of medicine meeting

Information Technology: Computer terminals (7-8) each floor, Dept of Medicine and RMO “mess”.  Radiology is all digital.
